1. Sunetics International is hereby authorized to take and exhibit photographs of the above client before, during and after treatment. The present consent is given on condition that the said photographs shall be used for scientific purposes only, including publication in scientific and professional publications and exhibitions to scientific and medical audiences. I hereby expressly waive any and all claims I might at any time have or pretend to have against said companies or individuals using my photographs, in any matter whatsoever relating to the said photographs as stipulated above.
2. I give permission for my photographs to be used for marketing purposes. Although the photographs and accompanying material will not contain my name or other identifying information, I am aware that I may or may not be identified by the photographs themselves.
